The Banns of Marriage Application Form, is attached below.

 

Please save this file by:-

 

Right clicking on the document

 

then clicking "Save target as" (in Internet Explorer) or "Save Link As" (in Netscape)

 

and then browse to the appropriate drive and folder, then save the file.

 

The form can then be edited using Word.

 

Please then:-

 

1. Print the form off 
2. Complete  
3. Send to the Reverend Simon Brocklehurst, Saint Andrews Vicarage, Church Road, Ham, Richmond, TW10 5HG, enclosing: 
   1. Completed form 
   2. statutory fee (£37 in cheque made payable to the Reverend Simon Brocklehurst ) 
   3. and SAE for your certificate 

We will email you upon receipt of these to advise when the banns are being called and what you need to do.
